You (reply) REPLIED , (enclose) ENCLOSING an entry form, which I (fill up) FILLED UP and (return) RETURNRD without delay. Since then, however, I (hear) HAVE HEARD nothing and I (begin) BEGUN to wonder if my application (go) HAS GONE astray.
WOULD you please (check) CHECK that you (receive) HAVE RECEIVED it and if you haven't, please send me another application form. If, on the other hand, you (receive) HAVE RECEIVED my application but (not decide) HAVEN'T DECIDED whether to accept me as a student or not, I (be) SHOULD BE very grateful if you (tell) WOULD TELL me when I may expect to hear your decision. Finally, if my application (already / be) HAVE ALREADY BEEN refused, I (like) SHOULD LIKE to be informed as soon as possible because if I (not get) DON'T GET into your college, I (have) WILL HAVE to apply to another and the sooner I (do) DID this, the better chance I (have) HAVE of being accepted.

Dear Ladies and Gentlemen,
I wrote to you three weeks ago, asking about conditions of entry into your college. You replied, enclosing an entry form, which I filled up and returned without delay. Since then, however, I have heard nothing and I began to wonder if my application has gone astray.
You please check that you have received it and if you haven't, please send me another application form. If, on the other hand, you have received my application but haven't decided whether to accept me as a student or not, I should be very grateful if you would tell me when I may expect to hear your decision.

Finally, if my application have already been refused, I should like to be informed as soon as possible because if I don't get into your college, I will have to apply to another and the sooner I did this, the better chance I have of being accepted.
